                                                                              1. W2-G and Win/Loss request forms are available on property at Guest Services with valid identification
                                                                              2. Use the online request form below
                                                                              3. The previous year’s win/loss statement is available on property at any of our promotional kiosks

ONLINE W2-G WIN/LOSS REQUEST FORM

Manual W2-G requests turned in at Guest Services will be forwarded to the Finance Department for processing and delivery.
All W2-G and WIN/LOSS copies will be forwarded to the guest via the address listed on the request form.

We apologize for any inconvenience, for reasons of security and confidentiality, copies of W2-Gs and Win/Loss statements are not available online.
Additionally, we cannot accept requests for win/loss statements via phone or email.

All requests received between January 1-October 31 are typically completed within 24 hours and forwarded to the guest.
All requests received between November 1 – December 31 are held and processed in the first 10 days of the following year.

Please allow up to two weeks for processing. Additional processing time may be required for W2-Gs if the player’s Oxford Rewards Loyalty Card was not inserted at the time of the jackpot. Please allow up to two weeks for a statement that must be mailed to you. See Guest Services for details.
